Broadening of TIME concept in wound healing

The TIME (Tissue, Infection/Inflammation, Moisture, and Edge) framework has been developed in 2002 by a group of wound care experts in an effort to standardize wound assessment and preparation of a wound bed for treatment.

When a patient enters the wound treatment regimen, these components have to be thoroughly assessed in order to optimize the treatment approach and facilitate fast healing of the wound.
​
We interviewed our Clinical Director Vicki Fischenich to find out how the TIME framework may have changed since the entrance of Enluxtra Smart Wound Dressing into the wound care market.


​Does it mean Enluxtra is making the TIME framework obsolete?
​

Ms. Fischenich: No, not at all. On the contrary, I like to point out that with Enluxtra’s arrival the TIME framework not only remains in place, it is also developed further.
It so happens that Enluxtra’s properties are extremely beneficial to the skin around the wound. Unfortunately, skin around the wound has been left outside of the TIME concept, yet it is just as important to healing process as the wound itself. It needs to be included in every pre-treatment assessment and be made part of the updated TIMES acronym where S stands for “skin”.

How did Enluxtra’s arrival affect the TIME framework?

Ms. Fischenich: The TIME framework remains important for selecting appropriate treatment for each particular wound based on wound assessment results. However, the challenge that’s always been there is just that: having to choose from a variety of treatment options appropriate for the condition of the treated wound.
With Enluxtra this challenge has been minimized as this product is suitable for all types of wounds at all healing stages.  As a result, the treatment is streamlined and clinicians have received a tool that can address all the components of the TIME framework simultaneously. 
Can you tell us a little bit about each component of the new TIMES framework and how Enluxtra affects each one of them?

Ms. Fischenich: Certainly. Non-viable tissue, such as necrosis or slough impede healing and prolong inflammation. Traditionally debridement takes care of the non-viable tissue. With Enluxtra there is the advantage of strong support of the natural autolytic process that helps eliminate the costly and painful debridement methods.

- Lingering infection and inflammation happen for a number of local and systemic reasons. The presence of pathogens in the wound has traditionally been dealt with by means of cleansing, and topical antibiotics. Persistent infections are treated with systemic antibiotics. Enluxtra’s ability to physically remove microorganisms from the wound and chemically bind the water necessary for their replication addresses infection without the need for additional procedures or medication.

- Exudate management presents the biggest challenge in wound care but moisture is an important factor in physiology of wound healing as it drives cell proliferation and epithelialization processes.  Conventional approach to maintaining moist healing environment is to rely on the correct dressing choice. The smart polymer technology allows Enluxtra dressings to adjust local functions in real time in response to the changes in wound microenvironment. This helps to optimize moisture balance at all times and in all parts of the wound.

- Wound edges contract during the normal healing process. A number of reasons may prevent wound closure and traditionally clinicians rely on the chosen wound dressing’s properties to reduce the factors impeding the migration of epidermal margins and contraction of wound edges. Non-traumatic wound contact surface of Enluxtra combined with its ability to prevent both maceration and desiccation puts this product at a clear advantage over other dressing choices.
​
- Finally, skin around the wound that is vulnerable both to mechanical damage and infection requires protection it is rarely given when traditional treatment options are used. Enluxtra treatment fully focuses on treating peri-wound skin - to the same extent it treats the wound. As a result, skin around the wound never dries out, never becomes macerated; it is protected from both harsh extrinsic factors and mechanical damage.

​Would you say that Enluxtra’s properties constitute exactly what the TIMES framework implies to be a preferred wound treatment?
​

Ms. Fischenich: Most definitely. Enluxtra is universally applicable to any type of wound and its technology supports the natural healing process while minimizing the factors that impede healing. It is definitely the product that addresses every component of the TIMES framework at the same time, and should be regarded as the most advanced and versatile wound care product on the market today. 

​What advice would you give to wound care professionals who use Enluxtra dressings?
​

Ms. Fischenich: Enluxtra dressings perform best when all the manufacturer’s recommendations are followed closely. Sick cells. Sickle cell disease sufferers swear by Enluxtra.

At only 6 months of age, Marqus Valentine was diagnosed with sickle cell anemia – a disorder that interferes with delivering oxygen to tissues and organs. Oxygen-starved tissues are slow to heal when there’s a break in skin or other injury; the resulting wounds are very painful and prone to frequent recurrence, so finding a product that can help manage such wounds with minimal hassle is essential for all sickle cell disease sufferers.
By age 32, Marqus has been hospitalized over a hundred times for multiple sickle cell complications. About 4 years ago, when their family home was flooded, Marqus got a small cut on his foot which turned into years of wound treatments and a search for a better solution to wound care. Marqus’ mother and registered nurse, Fran, turned to the internet to find something new. “We have used everything on the market, but I knew there had to be something new.” Fran found Enluxtra on Facebook and ordered free samples. ​

Hosted by Howard University Center for Sickle Cell Disease and Faces of Our Children, Inc., this year’s Stomp Out Sickle Cell Walk had the largest turnout ever, with more participants than the recent Chicago event. The District of Columbia has the highest incidence rate of babies born with sickle cell disease and trait compared to the 50 states. Attended by teams from around the country, this year’s walk also drew attention from news organizations and local, state and federal officials interested in improving the lives of those with the disease.

Go big! Or go home. Why peri-wound skin must be covered with Enluxtra to achieve accelerated wound healing.

We will never tire from repeating the peri-wound mantra: Peri-wound area is as much a part of the wound as the wound itself.

Covering all compromised peri-wound areas with Enluxtra dressing is the first and foremost thing you can do to improve and speed up the healing of your patients’ wounds.
Many of our users report that once they have adjusted the Enluxtra dressing size, they have witnessed a dramatic leap in healing progress.

Let’s rediscover why peri-wound coverage is so important
Compromised peri-wound areas are fragile and vulnerable to infection. If left unprotected, it is easy for bacteria to penetrate the micro-cracks characteristic to these delicate areas.
Once this happens you have a deteriorating wound that got infected and has grown in size.

But how do you know if the peri-wound area is compromised?
Any of the following conditions are a clear sign that protection is definitely needed:
  • Dry, scaly skin

When you observe any of these conditions, it is your responsibility to treat them as part of the wound and include them in total wound measurements when selecting the Enluxtra dressing size.

How to measure the wound for Enluxtra
We strongly recommend to add an extra step to your wound assessment routine: measure the compromised peri-wound area, and make sure that the selected Enluxtra fully covers this area and overlaps the healthy skin by at least 1 inch.

Lisa Williams: my father got a nasty wound!

Recently, through our Facebook page, we were introduced to Lisa Williams, the sole caretaker of her 81-year-old father.

November 2015: minor injury resulted in a bad wound

He’s struggled with a wound on his leg since November 2015. His injury was the result of a common, routine motion—he leaned forward from his chair, reached down, and fell. He hit the inside of his ankle against the hard chair leg, which prompted severe swelling, but progressed to a large, open wound.

As conventional therapies continued to fail and a scary prospect of leg amputation has started looming over, Lisa decided to take the matter in her own hands.
​In her research she has come across our Facebook page, convinced her father’s doctor to order free samples of Enluxtra, and committed to a consistent treatment plan. She was not going down without a fight, and we’re glad she didn’t.

Healing progress full on
Lisa posted photos of her father’s wound after every Enluxtra dressing change and our entire team followed the healing progress with excitement. Lisa had all the help she could get from us, with our Clinical Director Vicki Fischenich commenting on every posted photo and pointing out the signs of healing that may not have been evident at first.
Could it happen to you?
Wounds like that of Lisa’s father are not uncommon and can develop unexpectedly after a seemingly insignificant episode. We have seen many cases of chronic wounds that started out as a small scratch on the skin surface.

​What you as a medical professional should realize is that it is easy to prevent the wound from worsening and becoming chronic.


On high risk patients, such as elderly, diabetic, etc. we recommend treating even the smallest wounds with Enluxtra dressings. This will save you a lot of headache and your patient – a lot of unnecessary pain and frustration; it will also save a lot of time, money, and effort.

Yes! After three steady months, Lisa, her father’s wound care nurse, and their physician are practically pros at using Enluxtra. Despite a few initial setbacks, each dressing change revealed new, healthy skin, and a smaller wound. Lisa has updated us with incredible photos revealing her father’s wound. As of May 1 he no longer wears any dressings and his wound is completely healed!
